Summary:
As a working supervisor, leads and coordinates the daily operations of the Health Information Services (HIS) Team, which is comprised of Health Information Specialists and Panel Managers.
The Coordinator is required to be proficient in at least one of these roles (HIS Specialist or Panel Manager) at hire date and must be proficient in both within six months of employment.
The combined HIS team members perform administrative and certain clinical duties in accordance with clinic and departmental policies, agreements with community partners, state and federal policies, as well as HIPAA.
Health Information Specialist duties include coordinating, processing and monitoring various types of medical record requests; disseminating incoming patient records and communications in a timely and efficient manner; coordinating legal requests with risk management and legal counsel as needed; stays current with release of information rules and regulations; providing education on release of information as needed; coordinating medical record reviews; properly assisting requestors with access to patient health information (PHI), and utilizes electronic health records to respond to requests.
Panel Manager duties include collecting and organizing patient data before the patient visit by reviewing the electronic health record for specific data to ensure the productivity of that visit.
This data could include recommending health screenings (as determined by accepted guidelines); obtaining records from other providers; obtaining results of diagnostic testing or specialists consultations; communication with patients regarding pending referrals to specialists and/or pre-visit lab testing that needs to be completed.
Panel Managers also supply requested records to providers as requested during the patient's visit, as well as preparing, sending and following up on patient referrals to specialty providers.
Essential Functions/Major Duties and
Responsibilities:
A.
Health Information Services Coordinator 40% Coordinates the daily operations of clinic Health Information Services Team.
Performs duties with integrity and innovation to ensure completion and a high level of customer service delivery.
Directly supervises all Health Information Services staff.
Participates in interviews, selections, and hiring of employees in conjunction with Administrative Services Manager and Human Resources.
Performs supervisory functions including staffing, training and orienting new employees.
Assigns, reviews and oversees work; evaluates performance (Annual CHAT), completes time cards.
Creates, implements and supports quality assurance plan for program's success and uses data and statistical reports to anticipate, assess and address performance gaps and guide improvement.
Monitors program activities and procedures for compliance with rules and regulations.
Identifies training needed to enhance customer service delivery and staff satisfaction.
Responsible to perform a wide variety of tasks under the pressure of continuous interruption.
Addresses feedback and resolves conflict in real-time when possible.
Reports incidents to Administrative Services Manager and completes or assists team to complete RL6 reports.
Ongoing scheduled meetings with Administrative Services Manager.
Leads team as an excellent Customer Service role model.
Leads team by example with fiscal stewardship.
Identifies disciplinary concerns and reports to the Administrative Services Manager.
Provides input for program workflows, policies and procedures to assure grant requirements and program standards are met.
Provides budgetary consideration input to the Administrative Services Manager for annual budget.
Evaluates program equipment and computer systems to assure adequacy for delivering services.
Assumes responsibility for professional development.
Reports allegations of abuse, neglect and related incidents according to laws and guidelines.
Recognizes and educates staff that managing patient care is a team effort that involves clinical and nonclinical staff (e.

B.
Health Information Services Duties 55% Evaluate incoming medical records and attach to electronic health record for distribution to the appropriate party for processing.
Oversee the validation and certification of requests for patient health information ensuring they meet compliance standards and patient authorization.
Ensure all releases of information are in compliance with the request, authorization, policies, and State and Federal/ HIPAA regulations and guidelines, including 42 CRF Part 2 as they apply to Mental Health and Chemical Dependency records.
Release records as appropriate for RiverStone Health Clinical Division.
Serve as a resource in answering patient, provider and employee questions and concerns relating to requests for health information.
Provide training for other departments or Satellite Clinics as needed.
Obtain and enter information into the State of Montana Vital Statistics database as needed for the completion of Death Certificates for RiverStone Health Clinics.
Generate invoices for attorneys and other organizations as appropriate for release of records.
Conduct risk review audits for insurance companies and HEDIS audits for Medicare, as requested.
Complete records requests for Worker's Comp and Social Security Disability determinations.
Send letters with provider notes and normal lab results to patients for all Main Clinic providers.
Work closely with RiverStone HIPAA Compliance Officer.
Chart preparation for scheduled provider visits including walk-in patients as requested by care team.
Determine appropriateness of scheduled appointments.
Print out immunization history from imMTrax.
Print out records from the Montana Prescription Drug Registry.
Extensive review of medical records as outlined in the Panel Manager Procedure document.
Accessing outside facilities' electronic health records for additional medical records.
Communicate pre-visit needs with patient.
Review the Clinical Decision Support System (CDSS) and review for compliance.
Maintains patient charts, including preparing all necessary documentation for appointments and signing, dating, and verifying completed documentation for completeness and accuracy.
Preparation, delivery and follow up of outgoing referrals.
Outreach to patients regarding care needed and testing follow-up.
